I give permission for a technician to enter the premises when I am not present to have the work done.

I do not give permission to have the technician enter the premises and will set up a time with them to have the work done.

I hereby request the above described work to be done by my Lessor, affiliated companies, and/or third parties. I understand that any and all portions of said work that the Lessor is not required to perform under the terms of the Lease between the Lessor and Lessee or extension there of will be my responsibility and I agree to promptly reimburse Lessor for any such charges or costs.

I agree that by submitting this form, I am authorized to do so and agree to hold harmless property owner and his employees and/or contractors in any event of loss or dispute.
